Description
This is a feedback questionnaire that applies to those who have participated in mobility. The questionnaire in question has 18 items, 15 grid type items and 3 open questions. The items are structured as follows:
1. Mobility preparation - items 1-6
- Questions related to the way of organization, involvement of the project team, information received etc.
2. Mobility participation - items 6-18
- Questions related to the impact of the mobility (regarding professional and personal development), courtesy of the partners in the country of destination, knowledge, skills and attitudes acquired as a result of the mobility.
Also, in this part, there are 3 open questions:
Item 15 - Have you experienced any serious problems during your Erasmus mobility? If so, please mention them:
Item 16 - What aspects of the mobility did you particularly like?
Item 18 - How do you think the ERASMUS+ scheme could be improved – regarding mobility projects?
Review
Strengths
It is very well structured and it has logical coherence.
Provides an overview of the mobility and collects concrete information
It has open questions, formulated pertinently, so as to gather complex information
The questionnaire also assesses the personal development component, not just the professional one

Weaknesses
A weak point of this questionnaire is that it is not anonymous, so the honesty of the answers collected is questionable.
The questionnaire in question applies very well to the project for which it was designed. The transferability potential concerns its structure more, instead of the acquisition of items:
A: mobility preparation (destination information, travel details, collaboration with the project department - if applicable).

Availability of partners in the destination country
Professional development
New knowledge, skills and attitudes acquired as a result of the mobility
In what sense do the acquired competences correspond to the need for institutional development
